New Account Application
To apply for an Affina account, complete the application, then have all account holders sign the application. Send the completed and signed application to Affina along with the opening deposit of cash and/or securities required to establish an account. You will be contacted upon receipt of your application.
Account Transfer Form (ACATS)
Use this form to transfer all or some of the assets in a brokerage account at another firm into your account at Affina. Simply follow the instructions and mail the ACATS form to us. We will contact your existing broker and make all the arrangements for you. The ACATS form cannot be used to transfer accounts holding only cash or cash equivalents.

Use this form to conveniently transfer cash and/or securities from your USAlliance Federal Credit Union account. Simply complete the form and mail it to us. We will contact the Credit Union and make all the arrangements for you

Stock Power
Appoints agent attorney powers to transfer stocks or bonds on the books of a company, with full power of substitution in the premises. This form may be used when mailing stock or bond certificates for deposit into your account. A properly completed stock power is required if a stock certificate received by Affina has not been signed.
